Lewistown, Pa. Hub and Mobilization Model

Lewistown, a borough in the county seat of Mifflin County, Pennsylvania, was the first US community to adopt the Hub and mobilization model in May of 2015. With a population of 8300, and led by the Lewistown Police Department, local human service providers formed a Hub table and met twice weekly in order to identify situations of acute risk and respond collaboratively.

 

The Global Network provided guidance and support in the engagement of the community as well as interactive on-line training and on site support for the launch of their table. Remote guidance and support continued through 2015.

Manitoulin Island, Ontario

Located in Lake Huron, Manitoulin Island’s population of 12,600 hosts two incorporated towns as well as 6 Anishinaabe First Nation communities. The United Chiefs and Councils of Mnidoo Mnising (UCCM) along with the UCCM Police, provided leadership in activating a situation table in Little Current in 2015 to better serve those at acute risk in their communities.

The Global Network provided advisory support and guidance to agency leads in the formation of the Situation table as well as the governing steering committees. All participants were led through our interactive eLearning program and on site support was provided for the launch of their table.

Greater Napanee, Ontario

Greater Napanee is located approximately 40 kilometres west of Kingston and the county seat of Lennox and Addington County. Its location and proximity to Kingston, shared resources, as well as their simultaneous interest in community safety and well-being, created the opportunity to join Kingston in forming a regional CSWB initiative. With situation tables in both Kingston and Napanee, both are under the banner of KFL&A Community Risk Watch.

Building on existing and established relationships in the community, The Global Network provided guidance and support in developing their own risk driven collaborative. All members of the table were mentored through our interactive e-Learning program and on-site support was provided to launch the table. Community Risk Watch Napanee continues to grow and our firm remains connected.

North Bay, ON

North Bay was Ontario’s first adaptation of the Saskatchewan Hub model in December of 2013. Gateway of the North and located in Ontario’s North East, North Bay has a population of 55,00 and long standing collaborations in the community. Enhancing those existing collaborations was a natural step, and the Gateway Hub was created. With 17 local partners and agencies involved, the collaborative began meeting twice to discuss situations of acute risk, and then collaborating on pro-active solutions and supports for individuals and families. The Gateway Hub has been involved in over 500 situations to date and has been a local model of best practice, hosting 12 Ontario delegations interested in the model. The Gateway Hub had also been recognized by the Ontario Association of Chiefs of Police in 2014, and selected for a community safety award, and in 2015 was announced as recipient of the community leadership award by the Ontario Municipal Social Services Association.

 

The Gateway Hub was led and supported by Global Network founder, Brent Kalinowski 2013-2015.The Gateway Hub continues to meet and support those at acute risk in the community.

Northumberland County, Ontario

Large in geography yet relatively small in population, the Northumberland Community Mobilization Coalition (NCMC) is among the first semi-rural applications of the Situation Table and related CSWB planning, and it spans the towns of Port Hope and Cobourg, the First Nations Community of Alderville, and vast areas of rural, village and hamlet environments along the central-eastern shore and ridge areas north of Lake Ontario.

The Global Network founders provided early guidance, continuing support and mentoring via the GN e-Learning system to support the development and launch of the NCMC model.
