Lewistown, PA

Lewistown, a borough in the county seat of Mifflin County, Pennsylvania, was the first US community to adopt the Hub and mobilization model in May of 2015. With a population of 8300, and led by the Lewistown Police Department, local human service providers formed a Hub table and met twice weekly in order to identify situations of acute risk and respond collaboratively.
